Write what happened, in plain words: what the child ate and how much, when they napped, diaper changes or potty visits, what they did, and anything the family needs to bring. Keep to what you saw. A daily report is a record of the day, not an assessment of the child.
Infant reports need more detail than older rooms: each bottle or feeding with the time and amount, every nap with when it started and ended, and every diaper change marked wet or BM. Add the activities you did together, like tummy time, stories or songs, and what the family should send tomorrow.
Short. A few lines of notes, or a handful of sentences, is enough. Parents read it at pickup or on their phone, so clear facts beat long paragraphs. If the template is filled in, the report is done.
There is no single national rule. Whether written daily reports are required depends on your state's child care licensing rules, and sometimes on the age group. Check your state's licensing regulations and your own family handbook.

Type the way you jot things down. Shorthand, typos, times and any order are fine: "came 8:30 happy, bf all, nap 1-3, 2 wet, painted, mom pickup 5:15, needs wipes". The writer puts the day in order (arrival, meals, nap, diapers, activities, pickup), fixes common spelling slips and turns each note into a full sentence without adding anything. A note it cannot read is kept as a teacher's note at the end, so you can word it your way.
