Daycare attendance form or weekly sheet?
Monthly sheet
One page per room per month. Best for subsidy and food program claims, billing part-time families and spotting patterns.
Weekly sheet
One page per week. Easier to use at the door with daily notes. Use our weekly daycare attendance sheet.
Sign-in sheet
Records arrival and departure times and who picked up. Many states require it in addition to attendance. Use our sign-in and sign-out sheet.
Keep records
Keep attendance records for as long as your state licensing rules, the CACFP food program and any subsidy program require.
What the totals tell you
- Days present per child, for subsidy and voucher billing
- Average daily attendance per room, for staffing and ratios
- Children who are often absent, so you can check in with the family
- Busy and quiet days of the week, for scheduling staff

Common questions
What is a daycare attendance form?
A sheet that records which children were present or absent each day. A monthly daycare attendance form has a row for each child and a column for each day of the month, with a total at the end.
How do I fill in a monthly attendance sheet?
Write each child's name once, mark P or A in the box for each day, cross out weekends and closed days, and add up the days present at the end of the month.
Is an attendance sheet the same as a sign-in sheet?
No. Attendance shows who was there each day. A sign-in sheet records times and who dropped off and picked up, which many states require too.
How long should a daycare keep attendance records?
It depends on your state and programs. Licensing rules, the CACFP food program and subsidy programs each set how long you must keep records, often several years.
Is this monthly attendance sheet free?
Yes. Add your children's names and download a monthly attendance sheet, or a blank one to fill in by hand.
