How to run a teacher evaluation that helps
Before the meeting
- Observe the teacher in the classroom at least twice, at different times of day.
- Ask the teacher to rate themselves on the same form first.
- Collect facts: attendance, family feedback, training hours.
In the meeting
- Start with strengths, with real examples.
- Pick two or three goals, not ten. Make each one something you can see.
- Agree on the support you will give: training, mentoring, materials.
After the meeting
- Both sign and date the form, and give the teacher a copy.
- Check in on the goals within 30 days.

Common questions
What should a preschool teacher evaluation form include?
The teacher's name, position, classroom, the evaluator and date, ratings for key areas such as classroom safety, teaching and learning, guiding behavior, family communication and professionalism, plus strengths, goals and signatures.
How often should childcare teachers be evaluated?
Many programs do a 90-day review for new staff, then a mid-year and an annual review. Short classroom observations between reviews keep feedback timely.
What rating scale works best?
A simple four-point scale works well: needs improvement, developing, proficient and exceeds expectations. It avoids a middle option and makes goals clear.
Can I fill this form in online and print it?
Yes. Rate each item, add strengths and goals, then print or save it as a PDF. You can also print a blank form to fill in by hand.
Is this teacher evaluation form free?
Yes. It is free, needs no sign-up and runs in your browser. What you type in the tool stays in your browser.
