FAQ

Common questions

What is a child emergency contact form?

A short form with the people to call if a child is hurt, sick or needs to be picked up: the parents, then backup contacts in order, plus allergies or medical needs.

How many emergency contacts should a child have?

At least two besides the parents, and three is better. Choose adults who live or work nearby and can come quickly.

Can an emergency contact pick up my child?

Only if they are also on the center's authorized pick-up list. Most centers ask them to show photo ID.

What are the cut-out cards for?

Fill in the small cards and keep one in the child's backpack or cubby and one in the field trip bag, so the key numbers are always with the child.

Is this emergency contact form free?

Yes. Add your details, turn the cards on or off and download a printable form.
